Contact form error when embedding on my website

Hello all, I've asked this question before, in the wrong area, so I'm coming to the community for assistance.

I've attempted to add my "Long Contact Form" from Podio to my website (if it makes any difference, it's a Weebly site) and I'm getting an error message as seen in the photo. How can I fix this? Is there something additional I need to do? BTW, I have added my website address to my Podio account as instructed, and still... Nothing! 

Any and all advice is welcomed.

  please open a ticket with support and they will guide through the procedure to set this up.
  This form is only used to support API developers.
